Ruquired Materials for Changing Personal Passport (i.e. altering passport issuing authority)

  1. Original passport and copy of both sides of I-94 or Geen Card
  2. Application forms(Column of Relative MUST be filled)
  3. Application forms(four copies) of Renewing and Changing Official Businuss Passport for abroad students and scholars, Xerox copy is acceptable but each copy must be signed)
  4. 3 copies of 2 inches official photograph (must be recent photograph)
  5. Certificate of studying or employment (Statement from advisor or department, copy of appointment letter)
  6. $40 Fee(money order or cash only) for changing passport($50 for expired passport)
  7. If applying through mail, please include 2 returning envelopes:
    1. one with $2.20 stamp and your mail address
    2. another one with $0.60 stamp and address of your previous Chinese Institution
  8. $5.00 mailing service fee(money order or cash)
  9. Changing and mailing are processed by two different offices, please include TWO money orders.

Notes:

  1. If you have unsolved financial or other affairs with your previous Chinese Institution, please contact them directly. And please send the certificate of the clearence of relation with your previous Chinese Institution.
  2. The plan of repayment must include detail steps, date, and amount. This column must has your signature.
  3. Every space of application forms should be filled clearly and include all required documents. Or your application can not be processed properly.
  4. Make your money order payable to CHINESE CONSULATE
